Procedures for Requesting Disclosure, etc. of Personal Data

We will respond to requests relating to the personal data in our possession, such as requests for notification of the purpose of use, disclosure, correction/addition/deletion of content, suspension of use, erasure, and suspension of provision to third parties (hereinafter collectively referred to as “Requests for Disclosure, etc.”) made by an individual or their agent using our prescribed method in accordance with the following procedures.

(1) Notification of the purpose of use of your personal data

We will respond to requests to notify you of the purpose of use of your personal data in our possession after first verifying your identity (or the identity of your agent making the request).
However, we may be unable to notify you of the purpose of use, in whole or in part, in the following cases:

  • ・When the act of notifying you or disclosing the purpose of use could harm your life, body, property, rights, and other interests or the life, body, property, rights, and other interests of a third party.
  • ・When the act of notifying you or disclosing the purpose of use could harm our rights or legitimate interests.
  • ・When it is necessary for us to cooperate with a state organization or a local government body in their execution of affairs prescribed by laws and regulations and notifying you or disclosing the purpose of use could impede the execution of the affairs concerned.
  • ・When the purpose of use of the data you have identified is clear from the matters we have announced concerning that data.

(2) Disclosure of the content of your personal data

We will respond to requests to notify you of the content of your personal data in our possession after first verifying your identity (or the identity of your agent making the request).
However, we may be unable to notify you of the content of your personal data, in whole or in part, in the following cases:

  • ・When doing so could harm your life, body, property, rights, and other interests or the life, body, property, rights, and other interests of a third party.
  • ・When doing so would significantly hinder the proper execution of our business.
  • ・When doing so would violate laws and ordinances

(3) Correction, addition to, and deletion of personal information

We will respond to requests to correct, make additions to or delete (hereinafter referred to as “correct, etc.”) the content of your personal data in our possession after first verifying your identity (or the identity of your agent making the request).
However, we may be unable to correct, etc. personal data in the following cases:

  • ・When the content of the personal data in our possession is correct.
  • ・When special procedures are required based on laws and ordinances.

(4) Suspension of use of personal information, erasure, and suspension of provision to third parties

We will respond to requests to stop using your personal data in our possession, erase that data or stop providing that data to third parties (hereinafter referred to as “stop using, etc.”) after first verifying your identity (or the identity of your agent making the request). Please be aware that we may be unable to provide services and transactions according to your requests if we have stopped using your personal data, have erased it or have stopped providing it to third parties, in whole or in part, based on your request.
We may be unable to stop using, etc. your personal data in the following cases:

  • ・When doing so could harm your life, body, property, rights, and other interests or the life, body, property, rights, and other interests of a third party.
  • ・When doing so would significantly hinder the proper execution of our business.
  • ・When doing so would violate laws and ordinances.

Documents to be submitted when making a “Request for Disclosure, etc.”

To make a “Request for Disclosure, etc.,” please print 、this application form, fill it out, place your seal, and send it to the address stated above enclosed with a copy of one of the following documents.

Documentation to verify your identity

Please enclose a copy of one of the following identity verification documents. If there is a description of your registered address, then please mask the corresponding part.

  • 1 Driver's license
  • 2 Passport
  • 3 Basic Resident Register Card
  • 4 Alien Registration Card
  • 5 Health insurance card
  • 6 Other official documents that can verify your identity

Making a “Request for Disclosure, etc.” through an agent

If making a “Request for Disclosure, etc.” through an agent, please enclose a copy of the following documentation as well as a copy of any of the documents listed in items 1 through 6 in the preceding paragraph.

When the agent is your legal representative

  • 1 Copy of family register
  • 2 Health insurance card
  • 3 Certificate of Registered Matters
  • 4 Other official documents that can verify the authority of legal representation

When the agent is mandatary

  • 1 Our prescribed power of attorney
  • 2 Documentation to verify the identity of the agent (The same documentation listed under “Documentation to verify your identity”)

How we respond to “Requests for Disclosure, etc.”

In principle, we will respond to your request by sending a sealed letter to the address you have indicated.
Please note that we may be unable to respond to your “Request for Disclosure, etc.” in the following cases. In that case we will notify you to that effect and inform you of the reason.

  • 1 When we are unable to verify your identity or the identity of your agent.
  • 2 When you have not filled in all the required items in the documentation.
  • 3 When the information that is the subject of the Request for Disclosure, etc., does not correspond to “Personal Information Subject to Disclosure” (*).
  • 4 When doing so could harm your life, body, property, rights, and other interests or the life, body, property, rights, and other interests of a third party.
  • 5 When doing so could significantly hinder the proper execution of our business.
  • 6 When doing so would violate laws and ordinances.

*Your personal data in our possession is your personal information constituting a collection of systematically structured information that we have the authority to disclose, correct, make additions to, delete, stop using, erase or stop providing to third parties, in accordance with your requests. However, personal information that corresponds to any of the following items is not Personal Information Subject to Disclosure.

  • a. Information that, by clarifying its existence, could harm your life, body, and property or the life, body, and property of a third party.
  • b. Information that, by clarifying its existence, could promote or induce illegal acts or misconduct.
  • c. Information that, by clarifying its existence, could pose a risk to national security, damage the relationship of trust with other countries and international organizations or cause a disadvantage in negotiations with countries and international organizations.
  • d. Information that, by clarifying its existence, could hinder the prevention, suppression or investigation of crimes, public safety, and the maintenance of order.

Fees for “Requests for Disclosure, etc.” and how they are collected

Requests for notification of the purpose of use and disclosure of the content of personal data:

*Please enclose 600 yen worth of postal stamps if you wish to receive the notification/disclosure via certified mail with delivery restricted to the addressee.

Requests for correction, deletion, and suspension of use:

About the handling of personal data acquired in connection with Requests for Disclosure, etc.

We will use your personal information contained in the application form and identity verification documents, etc., only within the scope necessary to confirm your identity (or the identity of your agent who made the request) and respond to the request. We will retain this information for 2 years and then responsibly dispose of it.
